Scholarship Claim Form

Scholarships not claimed or officially postponed by December 15, 2026 will be forfeited as previously noted in your scholarship award letter.

To receive your scholarship, please provide the following:

Verification of enrollment. (A copy of the letter of acceptance from the college you will be attending)
A copy of the receipt of payment confirming your acceptance of enrollment.

This form can be used by any student who has been awarded a scholarship by the Paramus Scholarship Show Fund in May 2026 to claim their scholarship.
